The presentation showcases a clinical scenario, where an unexpected posterior capsule rupture (PCR) happened early on during nuclear fragment removal. It highlights how the surgeon performed pars plana anterior vitrectomy as soon as the PCR was recognised, and later, without the need for any scaffold, performs lens removal safely and efficiently. Finally, the PCR is converted into a continuous posterior capsulorhexis and a Toric IOL is implanted in the capsular bag with reverse optic capture through the anterior capsulorhexis.
