ICE syndrome poses a potpourri of challenges for the anterior segment surgeon. In this case of near total iris deficiency, advanced cataract, and severe, uncontrolled glaucoma with ICE membrane occlusion of a glaucoma tube, we highlight the techniques of “membranorhexis"/capsulorhexis, cataract extraction, custom flexible artificial iris implantation, and ab-interno tube revision.
