A patient was diagnosed with phacolytic glaucoma. Urgent lensectomy via scleral tunnel was planned.However, a contracted anterior capsule was noticed during capsulotomy and switched to pars plana lensectomy. First, sulcus IOL was implanted and then lens was aspirated via vitrectome with sparing anterior capsule. Then, trabeculectomy was successfully done under the previously formed scleral tunnel.
