In this episode, Dr. Weikert will showcase a complicated cataract surgery quicksand case in a patient with a mature cataract, prior pars plana vitrectomy, deep chamber, and 4 to 5 clock hours of zonular weakness. This quicksand moment demonstrates the challenges faced when trying to support the capsule during cataract removal in the presence of an extremely deep anterior chamber and zonulopathy. It also highlights the use of intraoperative gonioscopy while managing a malposition capsular tension ring. The resilience demonstrated by the challenges of this case is remarkable!
